Client Update Form - This form is to update your information such as your address, phone numbers, emergency contact information, etc.

General Information Request Form - Use this online form to request general information about Mayfair and our services.

Boarding Registration Forms - Print and fill out both forms before dropping your pet(s) off for boarding. Includes Information about your pet regarding medications, food, and your emergency contact numbers. Having these forms filled out before your arrival will greatly expedite the registration process.

Drop-Off Physical Exam/History Form - Print and fill out before dropping your pet off for any physical exam. Please as detailed as possible. A doctor will call you before any major diagnostics or treatments are performed, unless you specify otherwise. Having this form filled out before your arrival will greatly expedite the registration process.

Nutritional Profile Form - During each yearly physical exam, all Mayfair clients receive free personalized nutritional counseling from our Veterinary Nutritional Advocate, Martha Singleton. Print and fill out this form to bring with you to your exam.

Anesthesia/Sedation Authorization Form - Print and fill out before dropping your pet off for sedation or anesthesia. Having this form filled out before your arrival will greatly expedite the registration process.

Glucose Curve Form - Print and fill out before dropping your pet off for a glucose curve for diabetes regulation. If your pet is currently on insulin, we request that you have fed your pet its breakfast and have given its morning insulin dose before dropping off between 7:30 and 8:00 AM. Your dog or cat will need to stay in the hospital until about 5pm.
